Experience: 3-4years of experience

- Refers patients, families and caregivers as appropriate to other clinical resources/professionals
- Implements, evaluates and modifies plan of care
- Integrates research and evidence based knowledge into clinical practice
- Takes a leadership role in the evaluation of the effectiveness of interventions
- Triage and apply a collaborative team approach in identifying, analyzing and resolving patient care problems
- Collaborate with patients to assess and identify needs, issues, care goals and resources in critical factors for achieving desired outcomes for discharge, post hospital recovery and health maintenance/wellness
- Maintain current knowledge of multidisciplinary resources, programs and services, referring patients for community resources as appropriate
- Triages incoming and outgoing patient telephone calls as appropriate
- Assists patients and families to integrate the impact of illness and recovery into their lifestyles
- Demonstrates knowledge of therapeutic intent based upon physicians medical plan and standards of care, medicine, and treatment.
